Job Summary
Nurse Practitioner (PRN) performing in-home health assessments. Conducts adult/geriatric assessments including medical history, diagnoses, treatment, health education, referrals, and documentation per Matrix guidelines. Administers point-of-care and laboratory testing as needed, collaborates with physicians, case managers, social workers, and family, and provides services at the member’s residence (home or Skilled Nursing Facility). Scheduling is flexible (Sunday–Saturday, 8 am–7 pm) with travel to members’ locations; potential pathway to full-time roles. Competitive pay tied to number of assessments completed, with monthly earnings ranging from approximately $1,800 to $11,500 based on state and activity, plus mileage reimbursement and benefits. Requires NP licensure, board certification, prescriptive authority where needed, and the ability to travel up to ~80–90% of the time; driver’s license and auto insurance required. Required Qualifications
- Master’s Degree or commensurate NP experience and satisfactory NP licensure
- Current NP licensure in state of practice including prescription authority (or ability to obtain)
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
- Current BLS, ACLS or CPR certification
- 1 year of NP experience
- Ability to travel up to 80-90% of the time including overnight travel and weekends across assigned state and possibly neighboring states
- Valid state driver’s license
- Auto insurance coverage for state of residence (or ability to obtain)
Desired Qualifications
