Job Summary
Conduct adult and geriatric health assessments in members' homes or skilled nursing facilities, reviewing medical history, medication adherence, and social risks to identify care gaps. Administer point-of-care and lab-based testing as needed while collaborating with physicians, case managers, and caregivers on patient education and referrals. Provide clear documentation according to Matrix guidelines and protocols during visits lasting up to one hour. Schedule availability allows work from Sunday through Saturday between 8 am and 7 pm, with pay based on assessment completion rates ranging from $90 to $115 per visit. Requires Master's degree, current NP licensure with prescription authority, and ability to travel 80-90% of the time with a valid driver's license.
Required Qualifications
- Master's Degree or commensurate experience and satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP experience
- Able to travel up to 80-90% of the time for extended periods of time to include overnight travel and weekends across the assigned state and possibly neighboring states
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
