Nurse Practitioner (PRN) - In-Home Health Assessments

PRN Nurse Practitioner performs in-home Comprehensive Health Assessments (CHA) for adults/geriatric members, reviewing medical history, medications, social environment, and other risks during visits up to one hour. Responsibilities include administering point-of-care and lab testing as needed, coordinating with physicians, case managers, social workers, and family caregivers, providing education and follow-up, and documenting findings per Matrix guidelines. Services are delivered at members’ residences or Skilled Nursing Facilities with flexible scheduling (weekly through weekends) and potential transition to full-time roles.
