Job Summary
PRN Nurse Practitioner performs in-home Comprehensive Health Assessments (CHA) for adults/geriatric members, reviewing medical history, medications, social environment, and other risks during visits up to one hour. Responsibilities include administering point-of-care and lab testing as needed, coordinating with physicians, case managers, social workers, and family caregivers, providing education and follow-up, and documenting findings per Matrix guidelines. Services are delivered at members’ residences or Skilled Nursing Facilities with flexible scheduling (weekly through weekends) and potential transition to full-time roles. Strong communication skills, computer literacy with health record software, and the ability to collaborate with PCPs and care teams are emphasized.
Required Qualifications
- Master’s Degree or commensurate NP experience
- Current NP licensure in state of practice (prescriptive authority or ability to obtain)
- AANP/AACN/ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
- BLS/CPR certification
- 1 year NP experience preferred or new grads encouraged to apply
- Valid state driver’s license
