Job Summary
Nurse Practitioner (PRN) delivering in-home health assessments through Matrix Medical Network. Conduct adult/geriatric health assessments at members’ homes or Skilled Nursing Facilities, reviewing medical history, medications, social environment, and risks to produce comprehensive health insights. Administer and order point-of-care and lab-based testing, collaborate with physicians, case managers, social workers, and caregivers, provide patient education and follow-up, and document findings per Matrix protocols. Flexible scheduling ( Sundays–Saturdays 8am–7pm ) with PRN opportunity and potential transition to full-time in their home location or region. Sign-on bonus of $1,500 is noted. Requirements include Master’s degree (or equivalent experience) and NP licensure with prescriptive authority, board certification in relevant adult/gerontology/family/acute care specialties, current BLS/ACLS, minimum 1 year NP experience (new grads encouraged), valid state driver’s license and auto insurance, and proficiency with health-record software. Required Qualifications
- Master’s Degree or commensurate experience and satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP experience preferred, new grads encouraged to apply
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
