Job Summary
Nurse Practitioner (PRN) to perform in-home Comprehensive Health Assessments for adults/geriatric patients, reviewing health history, medications, social environment, and risks; administer point-of-care and lab testing as needed; collaborate with PCP, case managers, social workers, and family; conduct visits at members’ residences or SNFs; flexible scheduling (8am–7pm, Sunday–Saturday); per-assessment compensation with potential to transition to full-time; mileage reimbursement; ongoing education and licensure support.
Required Qualifications
- Master’s Degree or commensurate NP licensure
- Current NP licensure in state of practice with prescription authority or able to obtain
- AANP, AACN, or ANCC board certification in Adult/Adult Gerontology Primary Care, Family, Emergency, or Acute Care
- BLS, ACLS or CPR certification
- 1 year NP experience preferred (new grads encouraged)
- Valid state driver’s license
- Automobile insurance coverage (or ability to obtain)
- Strong computer skills and familiarity with medical/employee health record software
- Excellent verbal and written communication skills
