Job Summary
Conduct adult and geriatric health assessments in members' homes or skilled nursing facilities, reviewing medical history, medication adherence, and social risks while administering point-of-care and lab-based testing. Collaborate with physicians, case managers, and caregivers to manage referrals, provide health education, and ensure clear documentation per Matrix guidelines. Perform visits lasting up to one hour, scheduling availability between Sunday and Saturday from 8 am to 7 pm based on your preference. Master's degree, current NP licensure with prescription authority, and one year of experience are required, alongside BLS/ACLS certification and a valid driver's license. Transition to full-time roles available for those supporting home locations or specific regions.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
