Job Summary
Nurse Practitioner (PRN) conducting in-home health assessments (CHA) for adults/geriatrics, reviewing medical history, current medications, and social environment during visits up to one hour. Performs health assessments to identify care gaps, administers point-of-care and lab testing as needed, and collaborates with physicians, case managers, social workers, and caregivers. Services provided at the member’s residence (homes or Skilled Nursing Facilities) with flexible scheduling (including Sundays–Saturdays, 8 am–7 pm) and potential transition to full-time roles in the home location or region. Requires NP licensure, prescriptive authority, board certification in relevant adult or family specialties, CPR/BLS/ACLS, and strong communication and clinical documentation skills. Eligible for a $1,500 sign-on bonus with compensation tied to assessments completed; travel to member locations is involved, with mileage support and ongoing education/credentialing opportunities.
Required Qualifications
- Master’s Degree in Nursing (NP)
- Current NP licensure in state of practice with prescriptive authority or ability to obtain
- AANP/AACN/ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
- Current BLS, ACLS or CPR certification
- 1 year NP experience preferred, new grads encouraged to apply
- Valid state driver's license and auto insurance (or ability to obtain)
