Nurse Practitioner (PRN) - In-Home Health Assessments

In-home Nurse Practitioner assessments: Conduct comprehensive health assessments for adults/geriatric members at their residences or SNFs, reviewing medical history, medications, and social environment; provide health education, physician referrals, and case-management referrals; order point-of-care and lab testing; document per Matrix guidelines; collaborate with PCPs and care team; flexible scheduling with visits across Sundays–Saturdays; travel to members’ homes to complete CHA visits that improve quality of care and close care gaps. The role emphasizes independent clinical assessment, patient education, coordination with physicians and care managers, and ability to obtain/maintain NP licensure and prescriptive authority. Must have strong communication skills and computer proficiency; may involve licensure expansion to neighboring states as needed.
