Nurse Practitioner (PRN) - In-Home Health Assessments

Nurse Practitioner (PRN) conducting in-home Comprehensive Health Assessments (CHA) for adult/geriatric members, including medical history, health education, testing as needed, and coordination with PCP, case managers, and family members. Performs visits at the member’s residence (homes or Skilled Nursing Facilities), with flexible scheduling (weekend and evenings available) and potential to transition to full-time. Responsibilities include administering point-of-care and lab-based testing, documenting findings per Matrix guidelines, collaborating with physicians and care teams, and traveling to members' homes; requires NP licensure, master’s-level education, prescriptive authority, and board certification in relevant NP specialty. Strong communication, computer skills, and willingness to support ongoing education and credentialing are expected. The role emphasizes compassionate, empathetic patient care in home-based settings with mileage reimbursement and related benefits.
