Job Summary
Conduct adult and geriatric in-home health assessments, reviewing medical history, medication adherence, and social environments to identify care gaps. Administer and order point-of-care and lab-based testing as needed, while collaborating with physicians, case managers, and caregivers on patient education and follow-up. Provide services at member residences or Skilled Nursing Facilities, ensuring clear documentation per Matrix guidelines. Schedule visits Sunday through Saturday from 8 am to 7 pm based on availability, with an option to transition to full-time roles. Requires current NP licensure, Master's degree, and ability to travel 80-90% of the time.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP experience
- Able to travel up to 80-90% of the time for extended periods of time to include overnight travel and weekends across the assigned state and possibly neighboring states
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
