Job Summary
Nurse Practitioner – PRN provides hospice and palliative care services under the supervision of a hospice physician. The role involves home visits to perform comprehensive assessments (clinical, cognitive, psychosocial, and functional), determine prognosis for recertification, and collaborate with the interdisciplinary team to manage symptoms, provide counseling/education, and update care plans. Responsibilities include documenting findings in the EHR, communicating with the hospice physician and team, participating in quality improvement, and traveling to patient sites. Requires strong clinical assessment skills, familiarity with hospice regulations, and the ability to work within a team to ensure continuity and quality of care across the hospice experience.
Required Qualifications
- Master’s degree from an accredited Nurse Practitioner program
- Graduate of an accredited school of nursing
- Completion of NP program clinical requirements
- State-issued NP license
- Active Nurse Practitioner License
- Active Registered Nurse License
- Active Nurse Practitioner Furnisher License
- Certification: National Provider BLS – American Heart Association
