Nurse Practitioner (PRN) – Health Risk Assessments

Travel to and from patients' homes to perform comprehensive, non-invasive physical assessments. Review medical history, conduct medical reconciliation to ensure patient safety, and provide basic education on wellness and preventative healthcare. Document findings accurately and timely. This role involves completing in-home Health Risk Assessments for Medicare and Medicaid members in and around New Bedford, MA. No treatment or prescribing is required.
