Job Summary
Travel to and from patients' homes to perform comprehensive, non-invasive physical assessments. Review medical history, ensure accurate documentation, conduct medical reconciliation for patient safety, and provide basic education on wellness and preventative healthcare. This role focuses on in-home Health Risk Assessments for Medicare and Medicaid members in Northampton, MA, without treatment or prescribing. Qualified candidates must hold an active state NP license, national board certification, and BLS certification.
Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP license
- Active State NP license
- Active National board certification
- Current BLS certification
Desired Qualifications
- 12 months of experience as a paid advanced clinical provider
