Nurse Practitioner (PRN) – Health Risk Assessments

Travel to and from patients' homes to perform comprehensive, non-invasive physical assessments. Review medical history, ensure accurate and timely documentation, and conduct medical reconciliation to ensure patient safety and compliance. Provide basic patient education on wellness and preventative healthcare measures while interacting with Medicare and Medicaid members in an intimate, personal setting. This role focuses on completing in-home Health Risk Assessments without the requirement to treat or prescribe. Qualified candidates must hold an active state NP license and national board certification, with a master's degree or commensurate experience.
