Job Summary
Travel to and from patients' homes to perform comprehensive, non-invasive physical assessments. Review medical history, conduct medical reconciliation for patient safety, and provide basic education on wellness and preventative healthcare. Interact with Medicare and Medicaid members in an intimate setting to complete in-home Health Risk Assessments. This role does not require treating or prescribing. Qualified Nurse Practitioners with an active state license and national board certification are eligible to apply.
Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP license
- Active State NP license
- Active National board certification
- Current BLS certification
Desired Qualifications
- 12 months of experience as a paid advanced clinical provider
