Job Summary
Travel to and from patients' homes in Port Allegany, PA to complete in-home Health Risk Assessments for Medicare and Medicaid members. Perform comprehensive, non-invasive physical assessments and review medical history to ensure accurate, timely documentation. Conduct medical reconciliation to prioritize patient safety and compliance while providing basic education on wellness and preventative healthcare measures. This role focuses on intimate, personal interactions rather than clinical treatment or prescription. Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP license
- Active State NP license
- Active National board certification
- Current BLS certification
