Job Summary
Conduct adult and geriatric health assessments at members' residences or skilled nursing facilities, reviewing medical history, medication adherence, and social risks while administering point-of-care and lab-based testing. Collaborate with physicians, case managers, and caregivers to manage referrals, provide patient education, and ensure clear documentation per Matrix guidelines. Schedule visits between 8 am and 7 pm, Monday through Saturday, based on your availability to complete assessments for competitive pay.
Required Qualifications
- Master's Degree or commensurate experience and satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license and proof of adequate automobile insurance coverage for the state of residence
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
