Job Summary
Conduct adult and geriatric health assessments at members' residences or skilled nursing facilities, reviewing medical history, medication adherence, and social risks while administering point-of-care and lab testing. Collaborate with physicians, case managers, and caregivers to manage referrals, provide patient education, and ensure clear documentation per Matrix guidelines. Perform these visits Sunday through Saturday from 8 am to 7 pm, with flexible scheduling based on availability and potential for transition to full-time roles.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
