Job Summary
Nurse Practitioner – Primary Care – Telehealth, full-time on-site in Brooklyn, NY. Responsible for assessments and to recommend treatments, medications, or need for OT and PT, with geriatric patients in Assisted Living Facilities. Must hold NY NP certification and Medicare/Medicaid credentials; proficient in EMR and medical documentation; collaborates with on-site staff to develop and modify care plans and ensure quality patient care within a telehealth-enabled, on-site model.
Required Qualifications
- Current New York State Board Certified NP
- NY State Medicare and Medicaid credentials
- Experience working with geriatric patients
- Experience assessing, diagnosing, and prescribing treatments
- Strong computer skills and EMR experience
