Assess, diagnose, prescribe, and manage care independently for individuals and families across the lifespan, addressing physical health, chronic disease, mental health, and social determinants. Manage scheduled and urgent visits, diagnostics, medications, and follow-up while providing counselling, health education, and disease prevention strategies. Deliver care in the clinic and community, coordinating with physicians, specialists, and community services to connect patients with necessary resources.
