Job Summary
Provide and coordinate health care services to patients and their families, promoting maintenance of health and preventing illness. Deliver care during acute and chronic phases of illness while guiding and counseling individuals and families. Refer patients to other health care providers as appropriate. Record and update patient EMR in accordance with HIPAA and practice standards. Maintain a current, certified NYS Nurse Practitioner license with at least two years of experience in a family practice or urgent care setting. Work within a team-based Primary Care practice and Patient Centered Medical Home where NPs have ready access to precept cases with physicians.

- Current, certified NYS Nurse Practitioner license
- Certified NYS Nurse Practitioner license credential
Desired Qualifications
- Minimum of two (2) years' experience in a family practice or Urgent Care setting
- Previous experience working with Medent (EMR)
- Excellent working knowledge of chronic disease states
- Critical thinking skills
- Strong interpersonal and communication skills
