Nurse Practitioner, Primary Care/Family Practice

Assesses, diagnoses, plans and implements the medical care of patients in collaboration with a physician. Delivers high-quality, patient-centered care within the full scope of licensure; develops and manages an interdisciplinary plan of care that includes patient/family education, transitions of care, and coordination with physicians and other health care providers. Provides clear patient education using teach-back methods, ensures accurate documentation in the electronic health record, and maintains compliance with organizational policies, state regulations, and safety standards. Requires Master’s Degree in Nursing and California Nurse Practitioner licensure with applicable certifications; typically involves engaging in continuous professional development, leading care planning, and supporting process improvements to optimize patient outcomes. Location: Hidden Valley Lake, CA; Schedule: Full Time, Days; Licensure: NP, RN, DEA (as applicable), BLS; 5+ years of relevant clinical experience preferred.
