Job Summary
Provide primary care to high acuity patients in their home Monday through Friday. Deliver patient-centered care under an integrated, team-based, physician-driven model. Receive support from an assistant to manage appointments in select markets. Maintain a valid driver's license and good driving record. HarmonyCares operates home-based primary care practices in 14 states, employing over 200 providers to bring personalized healthcare to patients with complex needs who have difficulty accessing traditional care.
Required Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver's license and good driving record
Desired Qualifications
- Geriatric training/experience preferred
