Job Summary
Nurse Practitioner providing autonomous, primary care within PQCHC’s Integrated Health Services. Works as Most Responsible Provider for a dedicated client panel, diagnosing, treating, and managing acute and chronic conditions across the lifespan; orders and interprets diagnostics, prescribes medications, coordinates referrals, and conducts comprehensive health assessments. Collaborates with a multidisciplinary team to deliver team-based primary care, emphasizes prevention and health education, and supports clients experiencing health inequities related to social determinants of health. Maintains accurate EMR documentation and participates in quality improvement and professional development activities; bilingual English/French is an asset. Flexible hours including evenings; requires a valid vehicle and license. Master’s degree in Nursing with current CNO Extended Class registration (NP-PHC or NP-Adult) and a minimum of two years of NP experience are required.
Required Qualifications
- Master's degree in Nursing and current registration in good standing with the College of Nurses of Ontario (CNO) in the Extended Class (NP-PHC or NP-Adult)
- Minimum of two (2) years of experience practicing as a Nurse Practitioner, ideally in a primary care or community health setting
- Demonstrated experience or strong interest in working with priority populations: newcomers and refugees, older adults, 2SLGBTQ+ and gender-diverse clients, and health inequities related to social determinants of health
- Experience in a community setting desirable
- Demonstrated commitment to and knowledge of community-based health care
- Technology proficiency – PS Suite EMR, Microsoft Office Suite
- Linguistic profile: English/French is an asset
- Other languages an asset
