Required Qualifications

• •Master's degree in Nursing
• •Active and unencumbered CNS or Nurse Practitioner license in state of employment
• •Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric
• •Active NPI number
• •Active DEA certification if state allows prescription of controlled substances by APPs
• •Current collaborative physician agreement in place (All states except Iowa)
• •Ability to lift and move objects up to 50 pounds
• •Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
• •Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
• •Ability to write reports, business correspondence, and procedure manuals
• •Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
• •Strong written and verbal communications
• •Ability to understand, read, write, and speak English

• •Minimum of one (1) year nursing practice or equivalent experience (if providing Hospice Care)
• •Minimum of five (5) years nursing experience (if providing Palliative Care)
• •Ability to triage consults by patient acuity
• •Ability to effectively communicate patient and family needs to IDT
• •Ability to complete and submit required clinical documentation within 24 hours of visit completion
• •Ability to complete routine and emergency recertification visits on each patient entering their 3rd benefit period or beyond
• •Ability to complete routine and emergency assessments on each patient as indicated by departmental policy
• •Ability to record observations, treatments, and other pertinent information
• •Ability to communicate with Medical Director, Attending Physician, DCS and IDT as directed regarding findings from the visits and any measures to alleviate symptoms
• •Ability to provide physical and emotional care to support the patient and family in period of crisis
• •Ability to prevent unwarranted hospitalizations by meeting patient and family needs in the home
• •Ability to perform bedside palliative care patient/family evaluation in the patient's place of residence
• •Ability to assist with diagnosis, treatment, and management of acute and chronic health conditions
• •Ability to order, perform, and interpret laboratory and radiology tests within scope of professional practice
• •Ability to prescribe medications including controlled substances to the extent delegated and licensed
• •Ability to order treatments and durable medical equipment as indicated
• •Ability to perform other therapeutic or corrective measures as indicated, including urgent care
• •Ability to consult with palliative care physician or designees as needed
• •Ability to inform primary physician of services provided and collaborate with other physicians as needed
• •Ability to assist in all facets of care coordination for palliative care referrals
• •Ability to prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided
• •Ability to comply with applicable laws and regulations with respect to Collaborative Agreements
• •Ability to initiate reimbursement for services rendered
• •Ability to assist patients and families with goal setting related to treatment options and advanced care planning
• •Ability to provide training and continuing education for staff
• •Ability to assist in development of clinical practice guidelines/standards in support of quality palliative care
• •Ability to adhere to the practice of confidentiality regarding patients, families, staff and the organization
• •Ability to collaborate/communicate with IDT needs of patient for clinical and psycho-social interventions
• •Ability to assist in identifying the need for intervention of other IDT members
• •Ability to effectively communicate patient and family needs to IDT
• •Ability to complete and submit required paper work within 24 hours of visit completion
• •Ability to educate patient and caregiver regarding care of patient, disease process, dying process, symptom control, wound care, goal setting, treatment options, prognosis, and advance care planning
• •Ability to participate in monthly IDT meeting with market team
• •Ability to supervise nurse trainees, including graduate and advance practice nurses in field placements
• •Ability to participate in supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees
• •Ability to define goals for professional growth and participate actively in professional activities and organizations
• •Ability to engage in active and frequent self-care activities for personal and professional growth and longevity
• •Ability to articulate and embrace hospice philosophy

• •Ability to provide care and support in collaboration with the PCP and specialists
• •Ability to ensure compliance with practice standards and codes
• •Ability to collaborate appropriately with the assigned Compassus Medical Director

Desired Qualifications

• •Hospice, oncology, or home health and/or long-term care experience (if providing Hospice Care)
• •Clinical nursing care in oncology, hospice, geriatrics, intensive care (if providing Palliative Care)
• •Board Certification in Hospice and Palliative Care
