Job Summary

Conduct routine and emergency assessments, recertifications, and bedside evaluations for palliative and hospice patients across various care settings. Manage acute and chronic health conditions, order and interpret laboratory and radiology tests, and prescribe medications within delegated scope. Collaborate with the interdisciplinary team, medical directors, and primary physicians to coordinate care, alleviate symptoms, and prevent unwarranted hospitalizations. Provide education on disease processes, symptom control, and advance care planning while maintaining accurate patient records and completing required clinical documentation within 24 hours.

Required Qualifications

  • Master's degree in Nursing
  • Active and unencumbered CNS or Nurse Practitioner license in state of employment
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric
  • Active NPI number
  • Active DEA certification if state allows prescription of controlled substances by APPs
  • Current collaborative physician agreement in place (All states except Iowa)
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
