Job Summary
Nurse Practitioner role focusing on palliative and hospice care, providing assessments, disease and symptom management, and education to patients and families. Responsibilities include routine and emergency assessments, care coordination with the Interdisciplinary Team, orders for laboratory and radiology tests, prescribing medications including controlled substances, and potential after-hours or home visits. Requires a Master’s degree in Nursing and NP licensure with board certification in Hospice and Palliative Care preferred. Strong collaboration with Medical Director, IDT, and PCP; documentation within CMS guidelines; commitment to confidentiality and compassionate care across home-based palliative/hospice services.
Required Qualifications
- Active and unencumbered CNS or Nurse Practitioner license in state of employment required
- Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required
- Active NPI number
- Maintain active DEA certification if state allows prescription of controlled substances by APPs
- Board Certification in Hospice and Palliative Care is preferred, but not required
- Collaborative Agreements as required by state (All states except Iowa)
