Job Summary
Nurse Practitioner (NP) in Post Acute Care providing comprehensive health assessments, differential diagnoses, and treatment prescriptions under protocol in direct management of acute and chronic illness. Work autonomously and within interdisciplinary teams to promote wellness, safety, and high-quality patient experience in Skilled Nursing Facilities in Endeavors north region (Evanston, Swedish Hospital). Responsibilities include autonomous practice, managing acute/chronic conditions, and contributing to goals of care and advanced directives.
Required Qualifications
- Nurse Practitioner license in IL
- DEA license (Federal)
- Illinois Controlled Substances License
- Master's degree in specialty area
- BLS/CPR certification
- Minimum of three years NP experience
- Experience with advanced directives and goals of care
