Job Summary
Nurse Practitioner (NP) in Post Acute Care providing comprehensive health assessments and differential diagnoses, and prescribing treatments under protocol for acute and chronic illness. Work autonomously and within interdisciplinary teams; focus on wellness promotion, safety outcomes, and high-quality patient experiences across various settings. Requires IL NP licensure, DEA and Illinois Controlled Substance licenses, a Master’s degree, and BLS/CPR certification with a minimum of three years NP experience and experience with advanced directives and goals of care.
Required Qualifications
- License: Nurse Practitioner (APN) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
- Education: Completion of Master's level program in specialty area
- Certification: BLS/CPR certification required
- Experience: Minimum of three years Nurse Practitioner experience. Experience with advanced directive and goals of care.
