Job Summary
Nurse Practitioner (NP) in Post Acute Care providing comprehensive health assessments, differential diagnoses, and prescribing treatments under a patient-care protocol in direct management of acute and chronic illnesses; work autonomously and within interdisciplinary teams; focus on wellness, prevention, safety, and high-quality patient experiences across diverse settings; role includes promoting patient goals of care and advanced directives with a fixed schedule (Mon-Fri, some weekend potential) in a Skilled Nursing Facility setting in Evanston, IL; requires IL NP licensure, DEA and Illinois Controlled Substances licenses, a Master's degree, BLS/CPR, and a minimum of three years NP experience.
Required Qualifications
- Nurse Practitioner (APN) licensure in the State of IL
- Federal DEA license
- Illinois Controlled Substances license
- Master's level program completion in specialty area
- BLS/CPR certification
- Minimum of three years Nurse Practitioner experience
- Experience with advanced directive and goals of care
