Required Qualifications
- Nurse Practitioner license (APN) in IL
- Federal DEA license
- Illinois Controlled Substance License
- Master’s degree in specialty area
- BLS/CPR certification
- Minimum of three years NP experience
- Experience with advanced directive and goals of care
