Job Summary
Provide comprehensive health assessments and differential diagnoses for acute and chronic illnesses in post-acute care settings; prescribe treatments under established protocols; promote wellness and prevention of illness; work autonomously and within interdisciplinary teams as a resource and consultant; contribute to safety outcomes and high-quality patient experiences. Monday–Friday 8:00 a.m.–5:00 p.m. with potential weekend hours in the future and no evenings or holidays; location at Skilled Nursing Facilities in the North Region (Evanston, Swedish Hospital) under Endeavor Health; licensed in IL with DEA and Illinois Controlled Substances license; Master’s degree required; BLS/CPR certification required; minimum 3 years NP experience; duties include comprehensive health assessments and management of acute and chronic illness.
Required Qualifications
- Nurse Practitioner licensure in IL
- Master's degree in specialty area
- BLS/CPR certification
- DEA license for Controlled Substances
- Minimum 3 years NP experience
- Experience with advanced directive and goals of care
