Job Summary
Nurse Practitioner (NP) in Post Acute Care located in Illinois, providing comprehensive health assessments, differential diagnoses, and prescribing treatments under protocol for acute and chronic illness; autonomous practice within interdisciplinary teams; focus on wellness, prevention, safety outcomes, and high-quality patient experience in a hospital-system setting (Endeavor Health) with opportunities for professional growth and development. Requires IL NP licensure, DEA, and Controlled Substances licenses, Master’s degree, BLS/CPR, and at least three years of NP experience, with emphasis on advanced directives and goals of care. Regular hours Mon-Fri 8-5 (40 hours/week) with potential weekend hours in the future; no evenings or holidays.
Required Qualifications
- Nurse Practitioner (APN) license in Illinois required
- Federal DEA license required
- Illinois Controlled Substances license required
- Master's degree in specialty area
- BLS/CPR certification
- Minimum of three years Nurse Practitioner experience
- Experience with advanced directives and goals of care
