Job Summary
Nurse Practitioner (NP) in Post Acute Care providing comprehensive health assessments, differential diagnoses, and prescribing treatments under established protocols in direct management of acute and chronic illness and disease at skilled nursing facilities in Evanston, IL. Role emphasizes autonomous practice within interdisciplinary teams, wellness promotion, and high-quality patient experiences, with focus on advanced directives and goals of care. Hours are typically Monday–Friday 8:00 a.m.–5:00 p.m. (full-time), with potential future weekend hours; no evenings or holidays. Requires IL APN licensure, DEA and Illinois CS license, Master's degree, BLS/CPR, and at least three years NP experience; duties include autonomous decision-making, population health, and care management in diverse settings.
Required Qualifications
- Nurse Practitioner Licensure in IL
- APN Licensure in State of IL
- Federal DEA and Illinois Controlled Substance License
- Master’s degree in specialty area
- BLS/CPR certification
- Minimum of three years NP experience
- Experience with advanced directive and goals of care
