Job Summary
Manage a clinical caseload of ESRD and pre-ESRD patients within correctional, home dialysis, and nursing home/rehab settings. Perform comprehensive history-taking, physical assessments, and treatment planning while monitoring dialysis patients in partnership with nephrologists and facility staff. Prescribe medications in accordance with Florida Board of Nursing laws and ensure timely follow-up in Electronic Health Records Systems to comply with billing and Department of Corrections goals. Educate patients on disease processes, dialysis management, and wellness promotion while collaborating with multidisciplinary teams to ensure continuity of care. Contribute to new service development, quality improvement initiatives, and the Pre-ESRD Education Program. Lead and support the dialysis nursing team to maintain high standards of care. Work independently and as part of a multi-disciplinary team, with a schedule of three times per month.
Required Qualifications
- Master's degree in nursing from an accredited school
- Current RN License
- active Nurse Practitioner certificate
