Job Summary
Provide direct clinical services in accordance with health center policies and protocols while utilizing the Patient-Centered Medical Home model of care delivery. Assure access for continuity of care through appropriate referrals for sub-specialty and inpatient services and prompt follow-up of patients discharged from inpatient settings. Engage population health strategies to identify and address health needs among various segments of the primary care patient panel and make optimal use of the electronic medical record system. Collaborate with the practice team to prepare for office visits, manage high-risk and complex conditions, and actively engage in ongoing provider panel management activities. Scan patient records for gaps in care to ensure appointments are scheduled and indicated services are delivered. Work some non-traditional hours and travel between clinical sites in Springvale and Biddeford, Maine.
Required Qualifications
- A Master's degree in Nursing
- An unrestricted Maine license to practice as an adult, family, or pediatric Nurse Practitioner
- A U.S. Drug Enforcement Agency (DEA) license
- A minimum of one to two (1-2) years of experience in a community health or primary care practice
- A Master of Science in Physician Assistant Studies
- An unrestricted Maine license to practice as an adult, family, or pediatric Physician Assistant
- A U.S. Drug Enforcement Agency (DEA) license
- Compliance with NCCPA standards
- A minimum of one to two (1-2) years of experience in a community health or primary care practice
- Working knowledge of the core concepts of: Evidence‐based practice
- Working knowledge of the core concepts of: Social and behavioral determinants of health
- Working knowledge of the core concepts of: Population‐based care
- Working knowledge of the core concepts of: Integration of medical, behavioral health, and dental care
- Working within an electronic medical record
- Demonstrated respect for cultural diversity in serving patients and families
- Ability to apply critical thinking skills in performing patient assessment and care
- Willingness to work some non‐traditional hours
- Ability to travel between YCCAC/Nasson Health Care clinical sites
