Nurse Practitioner/Physician Assistant

Partner with the facility's therapy department to provide the best possible rehab care in a sub-acute setting. Collaborate with the patient's primary care team to ensure optimal outcomes, focusing on improving patient functionality and satisfaction in senior living communities. Coordinate care with medical providers, therapists, social workers, nurses, and families while providing on-site pain management to reduce re-admission rates. Perform timely charting and documentation, which can be completed from home. Work in-person at skilled nursing facilities with a flexible schedule, including full-time and part-time options. No on-call rotation is required, allowing evenings and weekends to remain free from interruption.
