Job Summary
Psychiatric Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) providing behavioral health and psychiatric medication management services within skilled nursing and senior living communities. Performs routine patient rounding, evaluation, and ongoing management of mental health conditions, collaborating with interdisciplinary teams to support residents’ emotional and cognitive well-being. Works with facility partners to ensure continuity of care, optimize treatment plans, and promote a therapeutic environment under a value-based approach emphasizing proactive care and improved quality outcomes. Required Qualifications
- Current NP or PA licensure in the state of practice
