Nurse Practitioner/Physician Assistant Primary Care

Job Summary
Provide primary-care services in long-term care facilities within family medicine, geriatric, or internal medicine scopes per site protocols. Review and sign MARs and orders for the facility, including phone orders, DME, death certificates, and plans of care, and manage labs within 24 hours. Perform narcotic prescription management per policy. Provide on-call triage per the triage protocol and participate in facility scheduling. Complete documentation and submit billing records within 48 hours of service; maintain open communication with facility staff and administrators. Ensure adherence to HIPAA and regulatory standards, and coordinate with clinical supervisor before refusing requests. Maintain patient confidentiality and adhere to EMR practices. Training and credentialing as required for facility-based primary care in skilled nursing/assisted living settings.
Required Qualifications
- Masters degree in Nursing or PA program from an accredited institution
- Current and valid license to practice medicine in the state
- Licensed as a Physician Assistant (PA)
- PA national certification examination passed or equivalent
- Specialization in geriatric, family medicine, or psychiatry desired
