Job Summary
Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care within skilled nursing and senior living communities. Duties include routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and collaboration with facility partners. Emphasizes preventive, value-based care, and improved outcomes for residents and families. Required Qualifications
- Current NP or PA licensure in the state of practice
- Previous professional patient care experience
- Experience in geriatrics, family medicine, or internal medicine preferred.
