Job Summary
Provide primary care services to patients in an assisted living facility and skilled nursing facility. Conduct comprehensive health assessments, develop and implement treatment plans, order and interpret diagnostic tests, prescribe medications, provide patient education, and collaborate with physicians and other healthcare professionals to ensure coordinated care. Maintain accurate medical records, participate in quality improvement initiatives, and adhere to applicable laws and standards of practice.
Required Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky
- Master's degree in Nursing or Physician Assistant Studies
- Certification as a Nurse Practitioner or Physician Assistant
- Experience in primary care, preferably in a long-term care setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
- Ability to maintain confidentiality and adhere to ethical standards
