Job Summary
Nurse Practitioner/Physician Assistant Primary Care ALF will provide primary care services in an assisted living facility, collaborating with the medical director and healthcare team. Responsibilities include conducting initial and ongoing health assessments, developing and implementing care plans, prescribing medications and treatments, ordering and interpreting diagnostic tests, patient education on health promotion and disease prevention, ensuring coordinated care with other professionals, maintaining accurate medical records, and participating in quality improvement initiatives. Requires current NP/PA licensure in Kentucky, a Master’s degree in Nursing or PA Studies, NP/PA certification, and experience in primary care, preferably in an assisted living or long-term care setting; strong communication, teamwork, and critical thinking skills.
Required Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky
- Master's degree in Nursing or Physician Assistant Studies
- Certification as a Nurse Practitioner or Physician Assistant
- Experience in primary care, preferably in an assisted living or long-term care setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
