Job Summary
Provide primary-care services in long-term care facilities (skilled nursing and assisted living) in accordance with site protocols and standards for family medicine, geriatric, or internal medicine. Responsibilities include reviewing MARs and orders, signing orders and documentation within required timeframes, managing labs and narcotic prescriptions per policy, completing billing/charting with EMR and hard copies, coordinating with facility staff and administrators, giving patient-care instructions, adhering to HIPAA and regulatory requirements, and participating in on-call triage and facility-wide care coordination with the clinical supervisor. Eligible candidates include Nurse Practitioners with a Master’s degree in Nursing or Physician Assistants who have completed an accredited PA program and hold active licensure and required certifications; specialization in geriatric, family, or psychiatry is desirable.
Required Qualifications
- Masters degree in Nursing or PA program
- Current and valid licensure to practice (Nurse Practitioner or Physician Assistant)
- National certification/examination as applicable (Nurse Practitioner or PA)
- Specialization in geriatric, family, or psychiatry desired
