Job Summary
Provide primary-care services in long-term care facilities (skilled nursing and assisted living) per site protocols, encompassing family medicine, geriatric medicine, and/or internal medicine. Responsibilities include reviewing and signing MARs and orders, managing labs and narcotic prescriptions, completing necessary charting and billing within established timelines, coordinating with facility staff and administration, maintaining patient confidentiality per HIPAA, and adhering to regulatory standards. Must meet state licensure requirements for Nurse Practitioner or Physician Assistant, with specialization in geriatric, family, or psychiatry preferred. Expect on-call triage duties and adherence to triage policies; maintain open communication with facility teams and participate in ongoing supervision and policy compliance.
Required Qualifications
- Masters degree in Nursing or PA program with licensure
- Licensed as Nurse Practitioner or Physician Assistant in the state of residence
- Certification appropriate to role (e.g., national certs for NP/PA)
- Specialization in geriatric, family, or psychiatry desired
