Job Summary
Provide primary care services as a Nurse Practitioner or Physician Assistant in an assisted living facility. Conduct initial and ongoing assessments, develop and implement care plans based on patients’ needs and goals, prescribe medications and treatments as appropriate, order and interpret diagnostic tests, educate patients on health promotion and disease prevention, collaborate with other healthcare professionals to ensure coordinated and comprehensive care, maintain accurate medical records, and participate in quality improvement initiatives.
Required Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky
- Master's degree in Nursing or Physician Assistant Studies
- Certification as a Nurse Practitioner or Physician Assistant
- Experience in primary care, preferably in an assisted living or long-term care setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
