Job Summary
Provide primary care services to patients in an assisted living/skilled nursing facility. Conduct initial and ongoing assessments; develop and implement care plans; prescribe medications and treatments; order and interpret diagnostic tests; provide patient education and counseling on health promotion and disease prevention; collaborate with other healthcare professionals to ensure coordinated care; maintain accurate medical records; participate in quality improvement initiatives.
Required Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant in Kentucky
- Master's degree in Nursing or Physician Assistant Studies
- Certification as a Nurse Practitioner or Physician Assistant
- Experience in primary care, preferably in an assisted living or long-term care setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
