Job Summary
Evaluate and manage patients in sub-acute rehabilitation and skilled nursing facilities alongside Board-Certified PM&R physicians. Collaborate with therapy departments to support functional recovery goals and coordinate care with primary providers, nursing staff, and family members. Manage on-site pain treatment plans to improve patient comfort and reduce hospital readmissions while maintaining accurate documentation. Build strong relationships with interdisciplinary teams to ensure continuity of care and contribute to measurable rehabilitation outcomes. This role offers flexible scheduling with no on-call responsibilities, allowing evenings and weekends off, within Michigan-based skilled nursing and sub-acute settings.
Required Qualifications
- Licensed Nurse Practitioner or Physician Assistant
- Active NP or PA certification
- Current Michigan licensure
- Strong communication and interdisciplinary collaboration skills
- Ability to work independently while contributing to a team-based rehabilitation model
- Interest in rehabilitation, senior care, pain management, and functional recovery
- Reliable transportation for travel between skilled nursing facilities
- In-person role within skilled nursing and sub-acute rehabilitation facilities in Michigan
- Part-time, Flexible scheduling available
