Nurse Practitioner / Physician Assistant

Job Summary
Provide evidence-based, patient-focused in-home and telehealth care as an Advanced Practice Provider (NP/PA) to patients with multiple chronic conditions (e.g., diabetes, hypertension, CKD, heart failure, COPD). Work within an integrated multidisciplinary care team, conducting comprehensive assessments, wellness exams, and post-discharge visits, educating patients and caregivers on disease management, and coordinating with PCPs and specialists. Participate in virtual care team meetings, quality assurance activities, and may practice in SNFs, ALFs, hospitals, and other clinical settings. Perform point-of-care testing, prescribe medications, order tests, and ensure timely documentation; travel to patient homes with reliable transportation and valid license. Requires active NP or PA license, board certification, DEA eligibility, and prior direct patient care experience; strong knowledge of HIPAA and EHR systems (e.g., Athena, Salesforce, Epic). Required Qualifications
- Active and unrestricted RN and NP license, or Physician Assistant license
- Board certification (NP: ANCC/AANP | PA: NCCPA)
- Current and unrestricted DEA certificate, or eligibility to obtain
- Minimum of two (2) years of direct patient care experience
- Field-based role requiring travel to patient homes, reliable transportation, valid driver’s license, and auto insurance
- BLS certification (supported during onboarding)
