Job Summary
Assess, evaluate, diagnose, and manage chronic conditions while providing care coordination for a geriatric patient population. Deliver patient care through assessment, planning, and implementation, collaborating with other disciplines to ensure safe and effective outcomes. Monitor quality care through performance improvement and provide leadership within the practice location. Perform clinician, teacher, and professional development responsibilities consistent with Florida Law. Requires a graduate of an NP or PA program with preferably three years of primary care experience, bilingual English/Spanish skills, and managed care coding expertise. Required Qualifications
- Graduate from a Nurse Practitioner or Physician Assistant program
- 3 Years of experience working in a primary care setting
- State of Florida License Requirements
- Managed Care, Medicare Advantage & HEDIS coding experience

- Preferably 3 Years of experience working in a primary care setting
- Bilingual English/Spanish
