Job Summary
Provide primary and palliative home-based care to a patient panel in the Greensboro/Winston-Salem, NC region as a Nurse Practitioner or Physician Assistant. Assess patients through physical examinations, review medical histories, develop impressions, diagnoses and treatment plans, and order diagnostic testing as medically necessary. Prescribe medications and medically necessary interventions, with supervision by a physician. Communicate effectively with office staff and clinical operations; attend quarterly medical staff meetings and foster trust with patients, families, and caregivers during visits. Requires board-certified NP or PA certification in NC and 1-2 years of primary care experience with geriatric patients. Expect a flexible, autonomous role with in-house clinical support and opportunities for growth within a expanding team. Required Qualifications
- Board Certified NP or PA certification in North Carolina
- Active NC NP or PA license
- 1-2 years of primary care experience with geriatric patients
- Ability to operate independently
